Confirmation and Youth Program Forms
If you are new to the Confirmation program, we hope you are looking forward to an exciting couple of years. If you are entering your second year, you are a veteran and know what to do. Moms and Dads, we need these forms to be completed so our young adults can fully participate in the Confirmation and Youth programs.
Download these forms to register for the Confirmation and Youth programs. You will need Adobe Reader to open and print these files. Adobe Reader is free and available at www.Adobe.com. Complete these forms and return them to the parish office no later than September 1, 2011.
Confirmation Registration Packets
Open and print the forms you need to register for Year One or Year Two. Complete a packet for each student registered in Confirmation classes.
Year One Confirmation Retreat Permission Form
There are the two waivers necessary for retreat registration. Please read the information and return the waivers to the Parish Office before October 7. Complete the waiver forms for each student registered in Year One Confirmation classes. The waiver form information includes a packing list (what to bring and what not to bring).
