Thank you for your willingness to be a Godparent and/or Sponsor! You will be a role model, living in harmony with the Catholic church, and promise to support the candidate as they embark on their journey of faith through Baptism or Confirmation. 

When selecting your sponsor, please consider the following:

• Your sponsor must be a registered, practicing member of the Catholic Church who has received the sacraments of Baptism, Eucharist, and Confirmation. If married, their marriage must have been celebrated in the Catholic Church.
• Your sponsor CANNOT be one of your parents.
• Your sponsor should be at least 18 years old and responsible for the task.
• We recommend that your sponsor be local, in order to help you grow in your faith.

Upon completion, please print the form and include the following: 

• Your signature
• Your pastor’s signature
• The parish seal (where YOU are registered)

Note: The name of the person to be Baptized or Confirmed (the Candidate) is at the TOP, and your name (the Godparent/Sponsor) is at the BOTTOM.
